Request for Catastrophic Leave Donations

We have received a request for participation in the Catastrophic Leave Donation Program from our University employee, Michael Alvarez, who has been determined eligible to receive sick and/or vacation leave credits.

If you wish to donate, please navigate to: https://hr.sonoma.edu/payroll/benefit-services/catastrophic-leave-donation-requests. The form can be signed electronically and returned to the Payroll & Benefits Office.
