Accident Forms

Please submit all completed forms to the Risk Management office within 48-hours of the time of the incident. Risk Management will review all reports prior to submitting them to the appropriate agency. PLEASE NOTE: If you are a SSU Employee, or the Supervisor of a SSU employee, reporting a work related injury or illness, please use the forms found on the Workers' Compensation website to reoprt the incident: http://www.sonoma.edu/erc/workers-compensation/incident-reporting.html
