Request for Administrative Review (pdf) — 2nd Level Appeal
This form is to be completed after notice that a citation which has been upheld at the 1st level appeal. Requests for Administrative Reviews must be received within 21 days of the notice of denied parking citation appeals.

Temporary Disability Parking Forms
Qualifying students, faculty, or staff who, due to a temporary or on-going disability, may apply for a temporary parking permit for parking in disabled zones on campus. For more information on the temporary disabled parking permits, please visit the parking permits page.
This form is to be used by qualifying students, faculty, or staff who would like to apply for a temporary parking permit. Applications should be returned to Disability Services for Students in Salazar Hall.
This document provides information regarding the acquisition and proper use of the permit.
Persons designated as "Campus Security Authorities" by the Jeanne Clery Act must use this form to report Clery reportable crimes to Police & Safety Services. This form is also used as the year-end report for Campus Security Authorities to whom no crimes were reported. For more information on Clery reportable crimes and Campus Security Authorities, please contact Sgt. Judy Mefferd.
This form is required when members of the public or persons involved in police incidents or traffic collisions would like to obtain a copy of a police report or other official document. Please review the circumstances under which state and federal laws permit the release of police records.

This form is required when planning certain types of events on campus. This form must be printed and filled out. Please consult with your event planner if you need further information. Completed forms should be returned to Conferences, Events and Catering, Salazar Hall.
Please submit all completed forms to the Risk Management office within 48-hours of the time of the incident. Risk Management will review all reports prior to submitting them to the appropriate agency.
