Address Changes for W-2 Mailing

The deadline to update mailing addresses for the 2015 W2 form is Friday, December 11, 2015. Changes made after this date will affect where the W2 is mailed by the State Controller's Office. Undeliverable W2s will be sent back to the Payroll and Benefits Office beginning February 2016.

Address updates can be made via Self Service in mySSU or by submitting the Employee Action Request form to Human Resources.

Replacing your W-2

If you did not receive your 2015 W-2, please verify with the Payroll and Benefits Office if your form was returned. Should you need to request a new form, please follow the instructions on our website. The Duplicate Wage and Tax Statement Request must be submitted to the State Controller's Office along with an $8.50 processing fee.

New Health Care Forms: 1095-B and 1095-C

Under the Affordable Care Act, the State of California, via the State Controller's Office, will issue Form 1095-C to its full-time employees with information about the health coverage officered, if any, to the employee and their dependents.

The state's health coverage providers will also issue Form 1095-B to employees who were enrolled in state-sponsored health coverage for at least one day during the preceding calendar year.

CSU 403(b) Tax Sheltered Annuity Program Transition

The CSU 403(b) Tax Sheltered Annuity (TSA) Program will transition to Fidelity Investments in April 2016. Updates and information can be reviewed at Fidelity NetBenefits' website.
